Então eu instalei o elastix e o iaxmodem vem no pacote.
To usando a versão 1.2 sendo que ele já ta na 1.5 não vou descartar erro na instalação.
O estranho que esse erro se resolve com um boot, mas volta depois de um tempo, não sei exatamente qual é o problema.

Não sei se tem alguma particularidade no elastix mas no asterisk puro to usando 1 modem na porta padrão, e ta registrando,
importante lembrar que no /etc/iaxmodem/ttyIAX1 o peername deve ser equivalente ao contexto criado no iax.conf.
Pra saber mesmo o que esta acontecendo seria ir ao fonte e verificar o que o asterisk esta reclamando :

ps : estou em uma maquina sem os fontes do asterisk
como vc esta iniciando o iaxmodem?
a instalação ficou ok ?
vamos procurar eliminar todos os pontos.
